- Print and complete by hand. Please send in with you child or post to: School Office, Shaftesbury High School, Weldbank Lane, Chorley, PR7 3NQ
Alternatively you can call the school office to order over the phone on 01257 249803 Option 1.
All orders must have cash payment provided on collection of the items ordered - we do not currently accept debit/credit cards or cheques.
