Key Stage 3 or 4 Medical Place Request
If you are requesting a Medical Place schools in addition to you completing one of the two forms above the Consultant will need to complete the Shaftesbury Supporting Medical Referral Form. Download it by clicking the image below - this MUST be completed and signed by the Consultant.
Please submit forms directly to the central mailbox sssmedicalreferrals@lancashire.gov.uk in order to be considered at the next medical panel - meeting dates available via this link.
